Impacts of Long Term Care Management on the Practices Related

PRACTICUM PROJECT PLAN

Long-term care management has a crucial impact on patient care and staff issues. Nurses need to develop a plan and carefully implement it so that they can take better care of the patients and also ensure that the best possible care practices are followed (Buchbinder, Shanks, & Kite, 2019). The management of Long-Term Care (LTC) and its practices are very important for any society and health care organisations. This is mainly due to reason that it helps in developing a better and healthier community that gives more emphasis on ensuring that the people get optimal care services. LTC management is especially important from the viewpoint of managing chronic and acute diseases among the patients. The key to providing better and more effective health care services is mainly dependent on the relationship between nursing staff and the patients. Such a relationship speeds up the recovery period and also helps in improving different skill sets of the nursing and healthcare staff. This study highlights and discusses the impact that long-term care management has on the practices relating to patients and staff issues.

Goal statement

The main goal of this project is to provide a recommendation to enhance management and leadership in LTC to meet organisational aims. In doing so, this project will explore the challenges faced by LTC management and leadership and determine its impact on the delivery of safe and patient-centred care and staff satisfaction in Long-term Care. The study focuses on several aspects of management, including leadership attitudes, human resources, regulatory and legal issues, financing, and budget. It will attempt to gain a clear understanding of how management and leadership shapes a culture that promotes safety, patient-centred care and healthy working environment on Long-term Care. According to Naidu (2019), a healthy working environment is one where employees have job satisfaction and are committed to achieving the organisation’s goal. It seeks to identify the existence of an environment that offers support and inspiration to its worker or the potential lack thereof. On the other hand, client-centred care is a recently evolving paradigm where the client’s autonomy, self-determination and empowerment are respected and influence the delivery of care (RNAO, 2019). Additionally, patient safety is largely determined by the lack or minimal harm caused to the client as well as adherence to existing best practice guidelines.

This study focuses on examining the impact of long-term care management on the practices and collaboration between patients and medical staff such as nurses in relation to improving the quality of the care services and speed up the process of recovery. In this research, attention has been given to addressing problems in the LTC management. Furthermore, this study focuses on issues such as lack of knowledge and qualification of the nursing staff, resistance to change and other such problems in LTC management (Cherry & Jacob, 2016). The data collected from different sources will be evaluated in relation to the various management programs that have been designed in such a manner that they focus more on the said issues and their impact on the quality of care services provided to the patients. This goal statement is the key to conducting the current project. It will guide the researcher in identifying and understanding different aspects related to the research topic and to develop different theories and concepts in relation to various parts of the practicum project.

Project objectives

The main objective of the project is to elicit both qualitative and quantitive data to develop a clear picture of the relationship between LTC management and the delivery of safe, effective, and efficient care to its long-care patients. It aims to:

1. rigorously analyse the various completed or ongoing activities that determine or support management accountability, effectiveness and efficiency

2. Identify existing post-hoc evaluation of previously spearheaded changes and improvement initiative by LTC management.

3. Identify challenges faced in sourcing, training and retaining qualified long-term care clinicians and support staff.

4. Identify budget restraints and allocation and how it impacts policies and programs

5. Determine the level of job satisfaction and joy in work among staff members.

6. Assess the level of expertise and ability to accept positive changes among the staff

6. Review quantitative measures related to job satisfaction such as absenteeism, attrition rate, and job-related injuries.

7. Look into the dimensions of patients’ experience namely: access to care, respect for patients’ preferences, coordination of care, information and education, physical comfort, emotional support, the involvement of family and friends, continuity and transition.

8. Review quantitive data on patient safety and harm caused to patients

The objectives of the project include proving that long-term care management is indeed effective in the practice that is related to patients and staff issues. Identification of strengths and weaknesses of the long-term management system is a way to improve the caring process between healthcare representatives and patients. This study also focuses on developing the standards that can help medical workers such as the nursing staff while creating and implementing a management plan. One of the crucial aspects that will be analysed in this project is regarding the way the management system to improving the health care practices is organised and managed.

Moreover, there is a need to specify a key feature of comprehensive and successful care management on the practices related to patient and staff issues. Lastly, it is imperative to figure out which problems may precisely prevent the process of management care from the implementation in the relationships between patient and healthcare representative. Additionally, the objectives have been set to make the outcomes of the plan more transparent and the project itself, beneficial for the long-term care management practice to dismiss barriers in the relationships between patient and doctor that always prevent the treating process from normal functioning.

An evidence-based review of the literature for project justification

Evidence-based research on the specific topic of management in healthcare is highly used for such projects. It is defined as highly beneficial in effective health care management systems. Scholarly sources applied to involve retrospective, experimental, and applied sociological studies are the basis for this project. The aim is to obtain reliable information about the phenomena or processes studied, about trends or contradictions in their functioning and development, so that these data can be used in the practice of social management of relations between healthcare providers and patients in LTC. Some scholars such as Eaton, Roberts, and Turner (2015) have contributed to the issue of long-term care management in patients and staff. The scholars mentioned that long-term care is imperative in healthcare since it supports the effective management of acute conditions that cannot be accomplished through short-term care (Eaton. et al., 2015). Researchers such as Kogan, Wilber, and Mosqueda (2016) provided a view on the long-term care management in the article that contributed to this plan. The authors in this regard stated that the patients and health care employees such as the nursing staff work together in order to improve the quality of care in LTC. Epstein et al. (2015) also mentioned in the evidence-based section that the inclusion of professional collaboration is paramount in harmonising cultural differences in a hospital setting.

Justification

The purpose of this project is to evaluate the role of long-term care management in the practices related to patient and staff issues. In times of significant technological development and an increased number of diseases, the need for advanced practitioners is becoming more and more crucial. However, in the face of rapid technological advancements, concepts such as effective communication and expression of psychological support for the patient have become outdated. Continuing with the same practices tend to have a negative impact on relationship between nursing staff and the patients. Due to this very reason, it is highly imperative for the authorities to implement methods for long-term care management practices so as to eliminate the barriers of relationships between the nursing staff and the patients.

Methodology

The study will mainly be a descriptive study utilising a mix of qualitative and quantitative data and a systematic review of the literature.

Qualitative data will be obtained through structured-interview, open-ended questions, observation and focus groups. The project will also run a small-scale survey to obtain quantitative assessment through the use of a validated tool to measure variables such as job satisfaction, perception and level of knowledge and skills. A review of the literature will also be utilised, with established exclusion and inclusion criteria. The articles are eliminated if they are locale settled. For focus groups and interviewing the subjects, the study will use purposive

sampling technique.

The methodology is a crucial part of the project since it shows how to implement the plan in the healthcare facility. The data collected from a range of sources, can be analysed using a wide variety of methods and techniques. Sources that will be used in the current study are both descriptive and analytical and will have been conducted on the topic of care management focusing particularly on improving the relations and practices between patients and the health care professionals or the nursing staff. The researcher further relied on a narrative review for assessing the validity as well as content of the data that was collected through the various sources in the current investigation. Main reason for using this method was to inculcate views of the nursing staff in the current research process. By collecting information from them the researcher can present a discussion on how the nursing staff can work towards taking better and effective care of the patients.

Another step is the conduction of investigation in the workplace, where all staff would be involved, that is called a poll. With this qualitative research method, in this study, the data will be collected on training results about long-term management that believed as a practical approach for the resolution of issues between patients and staff. Then, the study will use the facility approach to survey patients regarding whether the quality of service has changed after the training of health workers on the topic studied. Based on this, the data will be analysed manually for the number of positive and negative responses, which will allow concluding on the effectiveness of the plan and the presence of problems that it aims to solve.

The research resources will come from one gathering preliminary data on existing quality of life surveys, especially amongst LTC patients. To gain research validation, questionnaire template will be created for the early stages of data collection and will also recruit participants and collect data throughout the hospital throughout the practicum. According to Taherdoost (2016), the questionnaire is one of the most widely used tools to collect data, especially in social science research. The main objective of the questionnaire in research is to obtain relevant information most reliably and validly (Taherdoost, 2016).

Moreover, a wealth of information was gathered through literature reviews of surveillance programs for LTC management and execution of data analysis to identify trends in LTC management and how it impacts patient and staff issues. One will write a final report to synthesise the data, track current health policy trends, review similar programs to adapt for new care management programs by participating in constructing and designing questionnaires to measure the impact of LTC management on the quality of care for patients and staff practices. The study will assist in the construction of electronic data collection platform and evaluation of its efficacy, coordinate multi-party collaborations, and participate in follow-up data collections from polls.

The preceptor provided background materials supervised clinical activities and provided noteworthy feedback on completed tasks. Moreover, for receiving guidance and advice regularly, especially concerning LTC management and staff and patient issues. For example, the preceptor assisted in developing questions that will pull the information out of staff and patients.

Additionally, the preceptor connected with the relevant expert on questionnaire construction and information technology.

Resources

The project will be utilising search engines, research databases, local library resources,

local government websites and local publications to obtain statistical data as well as existing programs, policies and legislation related to LTC. Results from hospital-sponsored patient satisfaction survey and other quality indicator survey will be obtained from an organizational newsletter, incident reports and quality indicators from the Quality Department dashboard.

Questionnaires for a qualitative interview and focus group will be developed based on the literature review while the quantitative survey will make use of an industry-standard validated tool.

Formative and Summative Evaluation

The formative evaluation of the project estimates the positive and negative sides of LTC management practice and its outcomes on the service that is delivered by the healthcare workers to patients. Evaluation involves the differentiation of various methods that can achieve better results and objectives for the project. It will define whether a care-management approach should be used and taught among the healthcare workers or discontinued.

The summative evaluation summarised the positive and negative sides of the LTC management plan and its positive or negative impact on the practice. It also reveals whether the project uses evaluation and analytical approaches as well as rating scales and checklists and their roles. These approaches help in the assessment as well in offering guidance that can help in achieving the objectives of the project. Strategies are used in the formative method when the project is ongoing and in summative evaluation at the end of the project. The assessments used the developmental approach when the project is continuing in summative assessment at the end of the project. They will be used to determine whether the goal statement is achieved

Timeline

The project will take ten weeks. The schedule was formulated based on the weight of the work that needs to be done each week. It is fairly distributed, and it will help in completing the project within the time limit.

Conclusion

The LTC management plan assessed based on positive or negative outcomes concerning the practices that are delivered to the patient by the healthcare workers. The application of literary analysis and questionnaire among workers and patients are the main methods that are used for the identification of the results in this project. It is imperative to have the engagement of the nursing administration and management team when developing the plan to asses the care-management system. Moreover, the topic of the impact of LTC management on the Practices Related to Patient and Staff Issues needs more evidence-based research, and this study aims towards providing more evidence-based strategies that can improve practices in LTC management.
